Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses to examine the effect of treatment across trials with different levels of pragmatism.
Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. However, the usage of the term "pragmatic" is not uniform and its definition and assessment requires further clarification. The purpose of pragmatic trials is to guide clinical practice and policy decisions, rather than confirm the validity of a clinical or physiological hypothesis. A pragmatic trial should aim to be as close as it is to the real-world clinical practice that include recruitment of participants, setting, designing, implementation and delivery of interventions, determining and analysis results, as well as primary analyses. This is a key distinction from explanation trials (as described by Schwartz and Lellouch1) that are intended to provide a more thorough confirmation of a hypothesis.
The most pragmatic trials should not blind participants or clinicians. This can lead to an overestimation of the effect of treatment. Pragmatic trials should also seek to recruit patients from a wide range of health care settings so that their results can be compared to the real world.
Finally, pragmatic trials must focus on outcomes that matter to patients, such as quality of life and functional recovery. This is particularly relevant for trials involving surgical procedures that are invasive or have potential for serious adverse events. In addition to these characteristics pragmatic trials should also reduce trial procedures and data-collection requirements to reduce costs and time commitments. This can be accomplished by ensuring that their primary analysis is based on an intention-to treat approach (as described in CONSORT extensions).
Despite these requirements, many RCTs with features that defy pragmatism have been incorrectly self-labeled pragmatic and published in journals of all types. This could lead to false claims of pragmatism and the usage of the term should be standardised. The development of the PRECIS-2 tool, which provides an objective and standard assessment of pragmatic characteristics is a good initial step.
Methods
In a pragmatic study it is the intention to inform clinical or policy decisions by showing how an intervention could be integrated into routine treatment in real-world settings. This is distinct from explanation trials that test hypotheses about the cause-effect connection in idealized situations. Therefore, pragmatic trials might be less reliable than explanatory trials and may be more susceptible to bias in their design, conduct, and analysis. Despite these limitations, pragmatic trials can contribute valuable information to decision-making in healthcare.
The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatist). In this study the areas of recruitment, organization and flexibility in delivery, flexible adherence, and follow-up received high scores. However, the primary outcome and the method of missing data scored below the pragmatic limit. This suggests that a trial can be designed with effective pragmatic features, without damaging the quality.
It is, however, difficult to determine the degree of pragmatism a trial is since the pragmatism score is not a binary attribute; some aspects of a trial can be more pragmatic than others. Additionally, logistical or protocol modifications made during a trial can change its score on pragmatism. Koppenaal and colleagues discovered that 36% of the 89 pragmatic studies were placebo-controlled or conducted prior

Furthermore, a common feature of pragmatic trials is that researchers try to make their results more relevant by analyzing subgroups of the trial sample. Results
Although the definition of pragmatism does not require that clinical trials be 100% pragmatic, there are benefits of including pragmatic elements in trials. These include:
By incorporating routine patients, the results of the trial can be more quickly translated into clinical practice. But pragmatic trials can have their disadvantages. For instance, the appropriate kind of heterogeneity can allow the trial to apply its results to many different settings and patients. However the wrong kind of heterogeneity can reduce assay sensitivity, and thus reduce the power of a study to detect even minor effects of treatment.
A variety of studies have attempted to categorize pragmatic trials using various definitions and scoring systems. Schwartz and Lellouch1 developed a framework to distinguish between research studies that prove a physiological or clinical hypothesis as well as pragmatic trials that aid in the selection of appropriate therapies in clinical practice. The framework consisted of nine domains evaluated on a scale of 1-5 which indicated that 1 was more informative and 5 was more practical. The domains covered recruitment of intervention, setting up, delivery of intervention, flexible adherence and primary analysis.
The original PRECIS tool3 was built on the same scale and
프라그마틱 환수율 domains. Koppenaal and colleagues10 created an adaptation of this assessment, called the Pragmascope which was more user-friendly to use for systematic reviews. They discovered that pragmatic reviews scored higher across all domains, however they scored lower in the primary analysis domain.
This difference in the primary analysis domain could be explained by the fact that most pragmatic trials analyse their data in an intention to treat way, whereas some explanatory trials do not. The overall score was lower for pragmatic systematic reviews when the domains on organisation,

It is important to remember that a pragmatic trial doesn't necessarily mean a poor quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but this is neither specific nor sensitive) that use the term 'pragmatic' in their abstracts or titles. The use of these words in abstracts and titles could indicate a greater understanding of the importance of pragmatism, but it isn't clear if this is manifested in the contents of the articles.
